New Daily Pill Offers Hope for Patients Fighting High Cholesterol

A major change in heart health treatment has arrived. The United States Food and Drug Administration (FDA) has officially approved a new daily pill called Lipfendra, also known as enlicitide, to help people lower their cholesterol. This development is significant because high levels of LDL cholesterol—often called "bad" cholesterol—are a primary cause of heart disease, which remains the leading cause of death in the United States and across the world.

For decades, doctors have relied on a group of drugs called statins as the first choice for patients with high cholesterol. While statins are very common, they do not work for everyone. According to The Guardian Health, Dr. Puja Banka, a pediatric cardiologist at Merck, noted that seventy percent of patients are not reaching their recommended cholesterol goals using statins alone. Furthermore, some patients report unpleasant side effects from statins, such as muscle pain and weakness. The new drug, Lipfendra, is a type of medicine known as a PCSK9 inhibitor. Until now, these inhibitors were only available as injections, which made them difficult for some patients to use regularly. Clinical trials have shown that the new pill is highly effective. According to The New York Times, trials indicated that the drug could lower LDL cholesterol levels to 50 or 60, or even lower. MedPage Today reported that in two major studies, the drug reduced LDL cholesterol by 56% and 59% compared to a placebo—a pill with no medicine in it used for testing. While some patients in the trials reported mild side effects like dizziness or diarrhea, Merck stated that the side effects were generally very low, and in some cases, felt similar to taking a placebo.

This new medication is intended to be used alongside diet and exercise. In many cases, doctors will prescribe it to be taken in addition to statins to provide extra protection. However, it can also be used alone for patients who cannot tolerate statins. The drug is designed to be taken once a day, ideally after a period of fasting, and patients are advised to avoid eating for thirty minutes after taking the tablet.

Managing cholesterol is vital for preventing serious health events like heart attacks and strokes. New guidelines from the American College of Cardiology and the American Heart Association recommend that people at high risk should keep their LDL levels below 70, while those at very high risk should aim for levels below 55. Because many patients fail to meet these targets with current treatments, medical professionals are optimistic about this new option.

Regarding the cost, Merck has announced that the list price for the drug will be $10.50 per day, which totals over $300 per month. However, the company expects that the actual cost for many patients will be lower after insurance and other discounts are applied. The drug is expected to be ready for patients within a few weeks.

By removing the barrier of injections, this pill may help many more people reach their health goals and lower their risk of dangerous heart conditions.
